Przedmiot zamówienia obejmuje świadczenie i koordynację specjalistycznych usług w formie teleporad, które będą realizowane przy pomocy udostępnionej Wykonawcy przez Zamawiającego Platformy Koordynacyjno-Społecznej, zwanej dalej PKOS (opis założeń merytorycznych Platformy PKOS stanowi załącznik nr 1 do OPZ), w zakresie: a) Psychologa b) Dietetyka c) Gerontologa lub Psychogerontologa Szczegóły zostały opisane w OPZ.
