  • Submit signed contract (in duplicate if paper) plus annex 'Festlegung der vertragsgegenständlichen Arzneimittel' and signed declaration of reliability.
  • Documents can be submitted either in paper form (hand-signed, 2 copies) or electronically with qualified electronic signature via the DTVP portal.
