Przedmiotem zamówienia jest świadczenie usług medycznych w zakresie usług podstawowych dla pracowników Polskiego Radia S.A. w likwidacji i kandydatów do pracy oraz specjalistycznych usług medycznych dla pracowników Polskiego Radia S.A. w likwidacji i ich rodzin/partnerów oraz umożliwienie emerytom/rencistom Polskiego Radia S.A. w likwidacji i ich współmałżonkom/partnerom skorzystania we własnym imieniu i na własny koszt ze świadczeń specjalistycznych usług medycznych w zakresie objętym zamówieniem wraz z gwarancją ceny za pakiet medyczny w okresie trwania umowy. Szczegółowy opis przedmiotu zamówienia stanowi Załącznik nr 1 do SWZ. Pozostałe warunki wykonania zamówienia zawierają projektowane postanowienia umowy stanowiące Załącznik nr 2 do SWZ.
