Αντικείμενο της σύμβασης είναι προμήθεια, εγκατάσταση, παραμετροποίηση και λειτουργία ενός ψηφιακού συστήματος στις δύο οργανικές μονάδες του Γενικού Νοσοκομείου Θεσσαλονικης «Γ. ΓΕΝΝΗΜΑΤΑΣ-Ο ΑΓΙΟΣ ΔΗΜΗΤΡΙΟΣ» για 24 μήνες και για 1.000 χρήστες συνολικά το οποίο θα εξυπηρετεί τις προσδοκίες των επαγγελματιών υγείας του νοσοκομείου (ιατροί, νοσηλευτές/τριες, φυσικοθεραπευτές κ.λπ), των ασθενών με χρόνιες παθήσεις, καθώς και των φροντιστών τους, όσον αφορά την απομακρυσμένη παρακολούθηση και διαχείριση των παθήσεων τους, εφαρμόζοντας το μοντέλο της προληπτικής ιατρικής ακριβείας, με βάση τα καθημερινώς μεταβαλλόμενα προσωπικά δεδομένα υγείας και έκβασης της θεραπείας τους.
