Provision, implementation, and integration of a form module for digital creation, provision, and use of patient forms within an existing patient portal. The solution must enable structured data collection and integration into clinical and administrative systems, with particular attention to data protection, IT security, and interoperability. Excluded: the patient portal as a whole and a standalone electronic signature solution; the form module must only support integration of a signature solution provided by the client. A Master Service Agreement for IT-Services (based on a template) will be concluded.
